If you only want us to apply for a 501(c)3 for your ministry or charity, we will need the following information:

You will need to provide the name for the ministry or charity. 

Attach a copy of the state documents of incorporating. and your organizations adopted bylaws. Please provide the organizations EIN.

*The attachments must be no larger that 10mb

Will the organization make or has the organization made over $50,000 in your first three years of existence?

Does or will the organization have over $250,000 worth of assets the first three years of existence?

We suggest a minimum donation of $600 plus any applicable state fees for this service.
